• What happened
• The date and time it happened
• Who was involved
• How it affected you
• Your full name and address
• Your phone number or email address to contact you

We will:

• Acknowledge your complaint, in writing, within five working days

• Contact you promptly to discuss resolution of your complaint

• Investigate your complaint

• Inform you, in writing, of the outcome within 20 working days

If you are unhappy with our response, you are entitled to seek further help from agencies such as the Nationwide Advocacy Service or the Health and Disability Commissioner.
